They are sick

Respiratory failure needing support

Cardiovascular failure needing big vascular access, invasive monitoring, or vaso-active drugs

Reduced conscious level needing ABC support (ask "why is conscious level depressed ?")

Oligo-anuria requiring monitoring or therapy

A combination of the above

Therapeutic goals/monitoring
