Common Transfer Complications and Prevention

Accidental extubation — ensure ETT is firmly secured with tape and tied; confirm position before and after patient movement

Syringe pump failure / infusion interruption — check all pumps are charged and loaded with adequate drug volumes before departure

Hypoxia — calculate O2 requirements carefully; carry more than you think you need; have a self-inflating bag-mask immediately accessible

Hypotension — ensure vasopressors are running reliably; do not transfer haemodynamically unstable patients without full discussion and senior sign-off

Hypothermia — use transfer mattress cover and blankets; monitor temperature; warm the ambulance if possible

Communication failure — ensure mobile phone signal or radio communication with both sending and receiving units throughout transfer
